What Is an Adult Acquired Buried Penis?

Buried penis in adults is a condition wherein the penis becomes partially or completely concealed within the pubic skin or fat. The penis remains its normal size but appears short because it is buried. Men with this condition may experience:

  • Urinating problems
  • Bacterial and fungal infections
  • Phimosis (inability to retract the penis’s foreskin)
  • Sexual dysfunction
  • Psychological issues
  • Urethral strictures (narrowing of the urethra)

What Causes Buried Penis in Adults?

Obesity is the largest cause of this condition although some congenial conditions may also be a factor. Other factors that may contribute to or worsen buried penis include:

  • Bacterial or fungal infections
  • Scarring from previous procedures done in the male organ
  • Trauma to the genital area
  • Penoscrotal lymphedema – characterized by edema and painless enlargement of the scrotum and penis.
  • Lichen sclerosus – causes painful and itchy patches of thin, white and wrinkly-looking skin.

Diagnosing Adult Acquired Buried Penis

A urologist can diagnose a buried penis in adult men with a visual and physical exam. Your urologist will ask about your medical and family history to determine whether certain health conditions may be contributing to your case. Your groin area will be examined to check the extent of your condition and whether infections or other conditions are present.

The treatment plan is individualized for the patient and may depend on the underlying cause, extent of adult acquired buried penis, the patient’s overall health and existence of other conditions.

Treatment for Adult Acquired Buried Penis

Surgical procedures may be recommended to improve the functionality and visibility of the penis. You may undergo some medical tests weeks before your surgery to ensure you’re in good health and suitable for the procedure. Your doctor may also encourage losing weight and improving nutrition to help lessen possible complications during and after surgery. Surgery for a buried penis may include:

  • Scar tissue removal
  • Suction lipectomy – removal of fat cells

  • Escutcheonectomy – removal of excess fat above the pubic area.
  • Scrotectomy/scrotoplasty - removal of some scrotal tissue.
  • Split-thickness skin graft – taking a thin layer of healthy skin from another area of the patient’s body to cover areas of the penis with not enough skin.

Other treatments may include medications if there are infections in the genital area due to the buried penis. Psychological counseling may also be recommended to help address a patient’s mental health issues.
